Douglas County launches COVID-19 Phase 2 vaccine interest form

Douglas County health officials launched a COVID-19 vaccine interest form on Friday to help streamline vaccinating individuals who qualify to receive it under Phase 2 of the statewide plan.


The survey can be completed at dgcoks.org/vaccineinterestform. Douglas County residents can indicate their interest in receiving the vaccine and how they want to be notified once the vaccine is available.


Under Phase 2 of Kansas’ COVID-19 vaccine distribution plan, individuals who are 65 years and older and essential workers qualify for the vaccine. This includes teachers, child care workers, first responders, grocery store employees and food service workers.


The complete list of those who qualify under Phase 2 is available through the Lawrence-Douglas County Public Health website at LDCHealth.org/COVIDVaccine.


Vaccine supply has been extremely limited, said Business Systems Analyst Charlie Bryan, chair of the Vaccination Planning Unit under Unified Command, in a statement.


“This new form will help us identify which phase people would qualify for so that we can communicate effectively with them as doses become available for their priority group,” Bryan said. “We believe this will help with the notification process going forward.”


Phase 2 includes about 30,000 to 40,000 in Douglas County who qualify to receive the vaccine, according to a news release from Lawrence-Douglas County Public Health.

Senior citizens who may need help filling out the form can call Douglas County’s Senior Resource Center at 785-842-0543 and leave a message.


Douglas County has one public drive-thru clinic that will launch Jan. 29 for senior citizens. County health officials opened up 960 slots for those interested in receiving the vaccine, and both those appointments were quickly filled.


Health officials have previously said that once more vaccine doses are available in the county, then more appointment slots will become available.
